Terminal error:
java.lang.RuntimeException: Failed to update item set size.

what is the general reason for this problem?

Could you elaborate what you were trying to do? That is the error message you get when SQLContainer’s private method updateCount() gets a SQLException. Several SQLContainer methods use that functionality.
